Medication Review

A medication review performed by a pharmacy involves a comprehensive evaluation of a patient’s current medication regimen. This can include reviewing the patient’s medication list, identifying potential drug interactions or adverse effects, checking for correct dosing and administration, and making recommendations for necessary changes. A medication review aims to ensure that the patient is taking their medications safely and effectively and to prevent any adverse drug events.

How does it work?

MedCheck allows patients to bring in all their medications, supplements, and over-the-counter products to a pharmacy for a comprehensive review by a pharmacist. During the MedCheck, the pharmacist will review each product, including dosages and frequency, to ensure that the patient is taking their medications safely and effectively. The pharmacist will also check for potential drug interactions or adverse effects, and make recommendations for any necessary changes. The goal of the MedCheck program is to help improve patient outcomes by ensuring that they are taking their medications appropriately and to provide education and support for the safe and effective use of their medications.

Who is eligible?

Reference: Ministry of Health 

  • Patient must be an Ontario resident with a valid Ontario Health Card and currently taking a minimum of 3 prescription medications for a chronic condition.
  • Patient must agree to the MedsCheck service; the MedsCheck program is voluntary.
  • Patients are eligible for one MedsCheck Annual per year from the date of the original review.
  • Patients are eligible for a MedsCheck Follow-Up within the annual time frame under the following criteria:
    1. A patient is discharged from the hospital within the previous 2 weeks
    2. A pharmacist’s documented decision is based on:
      1. Significant changes made to an existing medication profile or the addition of new medication
      2. Documented evidence of patient non-compliance 
      3. Patients have changed their residence and transferred their prescriptions to a different pharmacy
    3. A referral from a physician (MD) or a nurse practitioner (NP)
    4. A planned hospital admission

Pharmacists are responsible for setting up an appointment with their patients, conducting a review of their medications, compiling a list of all their medications, having a personal meeting with the patient, providing a signed and dated medication review document, and submitting a claim for payment.
